Role Description The Case Manager – Aging & Family Support will provide coordinated support services to older adults and their families, focusing on assessment, planning, and ongoing case management. Day-to-day tasks include conducting needs assessments, developing individualized care plans, connecting clients with community resources, and monitoring service delivery to ensure that support remains appropriate and effective. The role also involves maintaining accurate documentation, collaborating with healthcare and social service providers, and offering guidance to families on navigating care options and support systems. This is a part-time, hybrid role based in Center District, Israel, with work performed both on-site and from home as needed. Qualifications
• Experience in case management, social work, gerontology, or related fields, with a focus on aging populations and family support.
• Strong skills in client assessment, care planning, and coordination of multidisciplinary services.
• Excellent interpersonal, communication, and active listening abilities, with a client-centered and culturally sensitive approach.
• Ability to maintain detailed records, manage multiple cases, and use digital tools for documentation and remote collaboration.
• Knowledge of local health, social services, and community resources relevant to older adults and families.
• Proven ability to work independently, exercise sound judgment, and collaborate effectively within cross-functional teams.
• Bachelor’s degree in Social Work, Psychology, Nursing, Gerontology, or a related human services discipline; relevant certifications are an advantage.
• Fluency in Hebrew required; proficiency in English and additional languages is an asset.
